Oska Health Raises €11 Million to Enhance Care for Chronically Ill Patients

Nearly half of chronically ill patients struggle with medication adherence, a challenge that Oska Health aims to address with its hybrid care model. The Frankfurt-based company has secured €11 million in Seed funding to expand its personalized health app, which combines AI technology and human health coaches to support patients between doctor visits.
The funding round was led by Capricorn Partners and SwissHealth Ventures, with contributions from investors including Revent, Calm Storm, LBBW Venture Capital, BMH, GoHub Ventures, and Aurum Impact. Oska Health’s approach targets high-risk, multimorbid patients living with conditions such as chronic kidney disease (CKD), diabetes, and hypertension.
Founded in 2022, Oska Health provides continuous care through certified health coaches who assist patients via video calls, chat, and a dedicated digital therapy app. These coaches help translate physicians’ recommendations into daily lifestyle and medication adherence, aiming to reduce complications and prevent hospitalizations. The service is offered free to insured members through contracts with health insurance providers.
This investment aligns with a broader trend in European HealthTech, where digital platforms are attracting early-stage funding to improve management of chronic diseases. Other recent funding rounds include Holi’s €3 million for obesity treatment expansion and Doctor.One’s €4 million to grow its chronic care model. Experts highlight Oska Health’s scalable model as a promising solution to the persistent gap in care between clinical visits.
Chronic diseases account for approximately 70% of healthcare costs in Germany, with millions affected across Europe. Oska Health’s CEO Niklas Best emphasizes the need for continuous support in everyday life, stating, “Chronic diseases cannot be managed in isolated doctor visits – they are decided in everyday life.”
